Travel Case Manager RN – 3x12 Days | ED-Based | Training Available

 

If you’re an RN looking to transition into or grow within Case Management, this is a great opportunity with a 3x12 schedule and supportive training environment.

You’ll work primarily in the Emergency Department, helping coordinate care and connect patients with resources.

 

Job Overview

  • Specialty: Case Manager RN (ED-Based)
  • Schedule: 3x12s (Day Shift)
  • Shift: 7:00 AM – 7:00 PM
  • Contract Length: 13 weeks
  • Start Date: ASAP (5/11)
  • Guaranteed Hours: 36 hours per week
  • Setting: Acute Care Hospital / Emergency Department

What You’ll Be Doing

  • Coordinate patient care and discharge planning
  • Assess patient needs and connect to resources
  • Work with multidisciplinary teams (providers, social work, etc.)
  • Support patients with complex social and medical needs
  • Assist in reducing unnecessary ED utilization
  • Navigate insurance and care coordination processes
