Description
Facility Name: LifePoint Rehabilitation Hospitals (Cerner/Omnicell)
Floor: Full hospital
Position Information:
· Facility Department/Cost Center: Pharmacy
Role (RN, Tech, LPN, PT, etc.): Pharmacist-in-Charge
Number of Openings: 1
Shift: 8am-4:30pm; on-call after hours; return to pharmacy for emergencies
Schedule
· Weekly Guaranteed Hours: 40
· Weekend Hours: On-call; return to pharmacy for emergencies
· Call Requirements: Yes, unless another pharmacist is delegated.
Required Certifications: Active, unrestricted pharmacist license in the facility state. Additional state-specific Pharmacist-in-Charge credentials (licensure/training requirements)
Experience:
· Required minimum of 1 year of hospital pharmacy practice experience (full time). If less than full time, will consider with additional years of experience and discussion of skills built during hospital tenure(s)
· Independent in at least one institutional electronic medical record and pharmacy information system, preferred experience with more than one electronic medical record and pharmacy information system.
· Required experience with hospital pharmacy automated dispensing machines; preferred experience with Omnicell.
· Preferred experience with regulatory compliance oversight with a background in audits, policies, and staff oversight.
Essential Functions:
Provides administrative leadership for daily pharmacy operations.
Identifies policy/procedure gaps and collaborates with Pharmacy Operations to resolve.
Ensures compliance with all applicable regulatory and accreditation standards (JC, CIHQ, CMS, DEA, State Board of Pharmacy).
Implement new services, procedures, and pharmacy automation as required.
Leads departmental Performance Improvement Programs.
Manage pharmacy staff including hiring, training, scheduling, coaching, and performance evaluations.
Develops and maintains technician and pharmacist schedules.
Adjusts staffing plans based on patient acuity and census.
Oversees purchasing and inventory management to ensure safe and cost-effective use of medications. Complete hospital medication inventory(ies) as required.
Accurately verifies and dispenses medication orders; oversees and approves technician work.
Monitors and evaluates all six critical processes of medication management to identify and mitigate risks.
Actively participates with physicians and clinical staff in quality and performance improvement initiatives.
Provides clinical pharmacy services, including anticoagulant monitoring, therapy optimization, and evidence-based drug utilization protocols.
Educates physicians, nurses, and staff with timely and accurate drug information.
Participates in medication reconciliation and documents clinical interventions in accordance with policy.
Maintains positive relationships with medical staff, administration, and patients.
Performs other duties as assigned.
