Description

Key Responsibilities:

Perform comprehensive health and physical assessments
Provide specialized wound, ostomy, and continence care
Develop and implement plan of care, treatment, and medication recommendations
Evaluate patient response and adjust care plans accordingly
Ensure accurate documentation and quality evaluation
Collaborate with interdisciplinary teams and manage referrals
Support patient and staff education, including in-services and training
Participate in quality improvement initiatives and promote best practices
Assist in pressure injury prevention and standards of care development
Maintain data security, confidentiality, and ethical standards
Support new employee orientation and continuing education initiatives

Required Qualifications:
Associate Degree in Nursing (ASN/ADN) from an accredited program
Active New York State RN License
Primary Source Verification from NYS Department of Education (NYSED)
AHA BLS Certification
Minimum 2 years of Med/Surg experience
Experience in wound and colostomy care

Required Certifications (One of the Following): CWOCN – Certified Wound Ostomy Continence Nurse (WOCNCB) CWON – Certified Wound Ostomy Nurse (WOCNCB) CWCN – Certified Wound Care Nurse (WOCNCB) CWS – Certified Wound Specialist (ABWM) WCC – Wound Care Certified (NAWCO) Qualifications: Experience in staff education, training, and clinical leadership Strong knowledge of wound care technologies, equipment, and supply selection
