Position Summary:

We are seeking an experienced RN Case Manager to join an acute care setting, providing comprehensive care coordination and discharge planning services. The ideal candidate will have a minimum of two (2) years of experience in case management or care coordination, with strong clinical knowledge and organizational skills. This role involves collaborating with interdisciplinary teams to ensure effective patient care transitions, optimize outcomes, and maintain compliance with hospital and regulatory standards.

Roles & Responsibilities:

• Coordinate patient care plans, including discharge planning and resource utilization.
• Collaborate with physicians, nurses, and interdisciplinary teams to ensure continuity of care.
• Assess patient needs and develop individualized care plans.
• Monitor patient progress and adjust plans to improve outcomes.
• Maintain accurate documentation in EPIC and ensure compliance with policies and regulations.

Skills:

• Strong knowledge of case management and care coordination in acute care
• Excellent clinical assessment and critical thinking skills
• Effective communication and interdisciplinary collaboration abilities
• Strong organizational and time management skills
• Knowledge of regulatory standards and discharge planning processes
• Proficiency in EPIC or similar Electronic Health Record (EHR) systems

Requirements:

• Minimum 2 years of RN Case Management or Care Coordination experience
• Valid New York State RN License
• BLS Certification (AHA)
• Flu Vaccine Required (No Exceptions)
• Certified Diabetes Care and Education Specialist (Preferred)
