Job Title: Registered Nurse - Case Management 
Location: Bronx, NY, 10457
Shift: Night shift but may need to cover days
Shift Length: 8 hours
Guaranteed Hours: 37.5 hours per week
Duration: 13 weeks

Position Summary:
Registered Nurse with 3 years of experience in acute care case management, responsible for coordinating and managing patient care across the continuum. Skilled in utilization review, discharge planning, and care coordination, ensuring appropriate resource use and timely transitions of care. Works closely with physicians, patients, and interdisciplinary teams to develop and implement effective care plans. Proficient in conducting PRI and Screen assessments and ensuring compliance with healthcare regulations. Holds current BLS and ACLS certifications with a strong focus on patient-centered outcomes and quality care delivery.

Roles & Responsibilities:

• Coordinate and manage patient care plans, ensuring appropriate utilization of resources in an acute care setting.
• Perform PRI and Screen assessments to support discharge planning and placement decisions.
• Collaborate with physicians, nurses, and interdisciplinary teams to ensure continuity of care.
• Monitor patient progress, review clinical data, and adjust care plans as needed.

• Educate patients and families on discharge plans, community resources, and follow-up care.
