Job Overview

The Integrated Case Manager is responsible for coordinating patient care across the continuum. This role focuses on assessment, care planning, discharge planning, and ensuring patients receive the right services to improve outcomes and reduce hospital readmissions.

Key Responsibilities

• Perform comprehensive patient assessments (medical, social, and functional)
• Develop and manage individualized care plans
• Coordinate care across inpatient, outpatient, and community settings
• Handle discharge planning and transitions of care
• Educate patients and families on treatment and available resources
• Collaborate with physicians and interdisciplinary teams
• Advocate for patients within their health plan and care system
• Monitor patient progress and adjust care plans as needed
• Connect patients with community and support services

Requirements

• Minimum 5+ years of case management experience
• Minimum 2+ years recent acute inpatient case management experience (REQUIRED)
• Strong experience with discharge planning
• Must be able to work independently with minimal training
• Strong communication, coordination, and problem-solving skills

🚫 Note:

• Telephonic case management, utilization review, or purely behavioral health experience will NOT be considered

Education

• BSN (Bachelor of Science in Nursing)
  OR
• MSW (Master of Social Work)

Licensure

• Active Michigan RN License
  OR
• Active LMSW (Licensed Master Social Worker)

Preferred Skills

• Knowledge of Medicare/Medicaid guidelines
• Understanding of social determinants of health
• Experience with community resources and care coordination
