Description
Expected to have knowledge of discharge planning and making referrals. Will typically be working on a MS or Tele unit and will split the floor with a Social Worker. Must be able to function independently on the unit and working collaboratively with an interdisciplinary team and patients for discharge planning. Expected to round on the unit with physicians/team.
Case load: Floors typically have 33-40 patients; Most floors are split with 1 CM & 1 SW; Caseload: 17-25 patients. Pilot Floors will have 1 CM & 2 SWs; Caseload: 12-17 patients. Need to be able to handle at least 20 patients. Typically placed on Med-Surg or Tele Floors following Internal Med and Family Med.
EMR: Allscripts - Do you require InterQual criteria or Milliman experience: Prefer some knowledge of CarePort, MCG, and/or InterQual. Prefer some Utilization Review experience. - Is this position for UR, discharge planning, or both?: Discharge planning - What areas do they cover: Inpatient
