Description
MANAGER INTERVIEW
Unit: Case manager
We are seeking a Registered Nurse Case Manager to provide home health services to patients throughout the Fox Valley and Oshkosh areas.
Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
● Registered Nurse license issued by the state in which the team member practices, and
● Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required, and
● A valid drivers license issued by the Division of Motor Vehicles
o Minimum Years of Experience required? 1
Education Required:
● Associate's Degree in Nursing.
● Skills required: Home Heath experience a plus, Epic experience a plus
● Excellent communication skills. Strong interpersonal skills and the ability to work with a variety of clients with patience and respect. Good organizational, analytical and problem solving skills.
● Demonstrated ability to work well with physicians and other professionals in a direct and positive manner.
● Proficiency in clinical skills with the ability to work under direction and make sound judgments.
● Ability to operate the necessary equipment to perform the job, i.e., FAX, computer, copier, telephone system.
● Ability to assess data reflecting the patient's status and the ability to interpret the appropriate information needed to identify each patient's requirements relative to their specific needs.
● Must have a tolerance for differences and an appreciation of multi-culturalism and diversity of the patients and their families.
Unit Information:
● Census? Average patients/day? 165 census 6 units a day
o Will you accept a first-time traveler? yes
● Patient Types/Common Diagnoses: Wound care, COPD, CHF, Diabetes, total joints, Comp assess, medication management, IV medications, picc line
Support within the Department:
● PT, PTA
● OT
● LPN
● MSW
● RN
● Technology/Equipment:
o EMR: Epic Remote Client, Rover
o IV Pump: Curlin
o Monitors: Laptop and mobile phone with hotspot
o Unit specific equipment: Home care bag and medical supplies
Scheduling:
o Weekend back up rotation: with other nurses about 1-5 weekends
o On call? If so, what is that schedule? 1 night a week and alternating weekends same as back up
o Holiday Expectations: none
o Should approval of RTO go through manager or will you allow the interviewing team (if applicable) to approve time off on your behalf? Rto team can but update manager
o Shift times: 8-430 Mon -Fri

I have worked with this company for 3, along with a few others, and they have been great! They seem to be very appreciative of their staff and always sending appreciative gifts to you. I can’t say much now that they have joined TotalMed, but am looking forward to see what develops.
Cons
Rates are not always the best, would be nice to have a few more eastern and southern state options, like Mississippi or North Carolina cities
