Description
π‘ Patient Assessment & Admission
- Perform initial and ongoing comprehensive assessments in the home setting
- Evaluate physical condition, home safety, and support system
- Develop individualized care plans based on physician orders
π Skilled Nursing Care
- Administer medications (oral, IV, injections)
- Perform wound care, dressing changes, and ostomy care
- Manage catheters, feeding tubes, and other medical equipment
π Monitoring & Disease Management
- Monitor vital signs and track patient progress
- Manage chronic conditions (e.g., diabetes, heart failure, COPD)
- Identify early signs of complications and intervene promptly
π Post-Hospitalization Care
- Provide care for recently discharged patients (post-surgical, illness recovery)
- Ensure continuity of care and reduce risk of readmission
ποΈ Care Coordination
- Collaborate with physicians, therapists (PT/OT/ST), and social workers
- Coordinate services like lab work, DME, and follow-ups
- Communicate patient status updates to the care team
π Documentation
- Complete accurate and timely documentation (often in systems like Homecare Homebase)
- Follow OASIS (Outcome and Assessment Information Set) guidelines for Medicare patients

- Educate on medication management, disease process, and lifestyle changes
- Teach caregivers proper care techniques
- Provide guidance on diet, safety, and symptom monitoring

- Assess home for fall risks and safety hazards
- Follow infection control practices in a non-clinical environment
π Travel & Independence
- Travel between patient homes (caseload-based visits per day)
- Work independently with strong clinical judgment
