Job Description:

Title: Solano County, California
Role: Physical Therapist - Home Health

Role Responsibilities:
• Required to prioritize start of care visits
• Required to supervise Physical Therapy Assistants
• Next day mandatory reporting of start of care visits
• Clinical notes and other documentation due within 48 hours of visit
• Maintain timely communication with supervisor, intake, and office
• Be generally responsive during normal business hours

Requirement: CA PT License and one year of Home Health experience
Territory: Solano County, Ca (Will be required to expand as needed)
Schedule: 40 hours per week, Saturday-Wednesday
Training: 3 full days, one in house and two in the field
Documentation System: Kinnser
Equipment Issued: iPad, Telecommunication One Talk
Mileage Reimbursement: ***/mile (from first to last patient)
Parking and Toll Reimbursement: Paid
Company Car: Option with a decreased bill rate

Bill Rate: ***
OT Rate: +*** paid after 30 points per week
Holiday Rate: +***
Weekend Rate: N/A
On-Call Rate: N/A
Call Back Rate: N/A

Productivity Requirement: 30 points per week
Point to Hourly Conversion: 1 = 1.33hrs
*Caseloads and visit mixes per day will equal to an average of 8 hrs per day and 40 hrs per week. Overtime must be pre-approved by the Clinical Supervisor.
