Position Overview

Marshfield Medical Center is seeking a compassionate and experienced Home Health & Hospice Registered Nurse (RN) to provide high-quality, patient-centered care in a home setting. This role focuses on managing chronic conditions, supporting end-of-life care, and delivering education to patients and families.

Schedule

🕒 Day Shift (8-hour shifts)
• Monday – Friday: 8:00 AM – 4:30 PM
• 2 on-call shifts per week
• Weekend coverage only during on-call rotation
• On-call weekend every ~6 weeks

Key Responsibilities

• Provide in-home care and hospice services for primarily geriatric patients
• Perform routine and OASIS assessments
• Manage chronic illnesses and complex conditions
• Administer medications, IV therapy, and wound care
• Deliver patient and family education
• Provide compassionate end-of-life care and symptom management
• Participate in daily huddles and interdisciplinary team (IDT) meetings
• Maintain accurate documentation using Cerner

Patient Population

Primarily geriatric patients with conditions such as:
• CHF, COPD, pneumonia
• Stroke, dementia, Alzheimer’s
• Cancer, kidney and liver disease
• Diabetes and chronic illnesses
• Post-surgical care and wound management

Requirements

✔ Minimum 1 year RN experience (Home Health/Hospice preferred)
✔ BLS Certification (AHA)
✔ Active RN license
✔ Strong critical thinking and independent decision-making skills
✔ Travel experience preferred

Required Skills

• Chronic disease management
• Complex wound care (including NPWT)
• Ostomy and central line care
• IV administration
• Patient and family education
• Hospice care: pain & symptom management, end-of-life support

Workload

• 5–6 patient visits per day
• 25–30 points per week
• Mix of routine and OASIS visits

Support & Environment

• 3 Certified Nursing Assistants available
• Administrator on-call support after hours/weekends
• Collaborative interdisciplinary team
