Description

🩺 Travel RN – Case Management | Days
🕒 Shift: Days | 4x10s (40 hrs/week)

💼 Key Responsibilities

Coordinate and manage patient care plans from pre-admission to post-discharge
Collaborate with physicians, nurses, and multidisciplinary teams
Act as a liaison between patients, families, and healthcare providers
Facilitate discharge planning and resource coordination
Monitor patient progress and adjust care plans as needed
Ensure compliance with hospital policies and care standards
Provide guidance and consultation on case management processes

✅ Requirements

Minimum 1 year of acute care case management experience
Strong communication, coordination, and organizational skills
Ability to manage multiple cases and prioritize effectively
Experience working with interdisciplinary teams

📜 Licenses & Certifications

Active Registered Nurse (RN) license (California or compact eligibility)
Case Management Certification – Preferred
BSN (Bachelor of Science in Nursing) – Preferred

📅 Schedule & Details

Day Shift: 8:00 AM – 6:30 PM (4x10s)
40 hours/week guaranteed
Weekend rotation required
No on-call requirement
