🩺 Travel RN – Case Management | Days
🕒 Shift: Days | 4x10s (40 hrs/week)
 

💼 Key Responsibilities

  • Coordinate and manage patient care plans from pre-admission to post-discharge
  • Collaborate with physicians, nurses, and multidisciplinary teams
  • Act as a liaison between patients, families, and healthcare providers
  • Facilitate discharge planning and resource coordination
  • Monitor patient progress and adjust care plans as needed
  • Ensure compliance with hospital policies and care standards
  • Provide guidance and consultation on case management processes

✅ Requirements

  • Minimum 1 year of acute care case management experience
  • Strong communication, coordination, and organizational skills
  • Ability to manage multiple cases and prioritize effectively
  • Experience working with interdisciplinary teams

📜 Licenses & Certifications

  • Active Registered Nurse (RN) license (California or compact eligibility)
  • Case Management Certification – Preferred
  • BSN (Bachelor of Science in Nursing) – Preferred

  • Day Shift: 8:00 AM – 6:30 PM (4x10s)
  • 40 hours/week guaranteed
  • Weekend rotation required
  • No on-call requirement
