Certification: BLS (AHA)
ACUTE CARE EXP. REQUIRED
Hospital Experience Required
No Call Required (potentially weekends)
Must have: candidates without these skills will not be considered for the role.
· Acute hospital case management experience 2+years
List typical procedures performed on unit(s):
· Discharge planning, chart review and patient screening
· Care Coordination
· Barrier mitigation
