🚨 Travel Registered Nurse (RN) – Home Health | Iron Mountain, MI 🚨

We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. This role involves managing a full caseload, including admissions, ongoing visits, and coordination of care across a diverse patient population. The ideal candidate is highly organized, independent, and experienced in OASIS documentation and home health workflows.

• Start: 5/25 | 13 weeks
• 8-hour days shift
• Recent Home Health experience (within 2 years)
• Strong OASIS documentation skills

Key Responsibilities

• Perform Start of Care (SOC), recertifications, and routine visits
• Complete OASIS documentation accurately and timely
• Manage full episode of care for assigned patients
• Provide skilled nursing care including:
  • Wound care
  • IV therapy and port management
  • Lab draws
  • Chronic disease management
• Coordinate care with physicians and interdisciplinary team
• Maintain timely and accurate EMR documentation (Epic preferred)
• Participate in on-call and weekend rotations

Patient Load & Travel

• Average 5–6 patient visits per day
• Travel may include up to 150 miles in a day (not daily)
