RN Case Manager – Acute Care | Mission Viejo, CA |

Location: Mission Viejo
Pay: Up to $2,727.10/week
Start Date: ASAP
Duration: 13 Weeks (Possible Extension)
Schedule: Day 5x8-Hour (08:00 - 16:30) + Weekend Rotation
Setting: Acute Care Hospital

Job Overview

We are seeking an experienced RN Case Manager with a strong background in acute care and care coordination. This role focuses on discharge planning, utilization review, and interdisciplinary collaboration to ensure optimal patient outcomes.

Key Responsibilities

Perform care coordination & discharge planning
Conduct patient assessments and develop plan of care
Manage appeals, denials, and utilization review
Coordinate DME, community resources, and follow-up care
Ensure compliance with CMS, HIPAA, OSHA, and Joint Commission standards
Collaborate with multidisciplinary teams across units (ED, ICU, Med-Surg)

Requirements

Active California RN License (must be active by start date)
Minimum 2 years acute care Case Management experience
Strong background in care coordination
Travel experience REQUIRED (first-timers not accepted) ❗
Experience with Epic EMR
Must be comfortable floating to nearby facilities (Laguna location)

Patient Ratio: 1:25

Dress Code: Turquoise scrubs or business casual
