Title: Registered Nurse (RN) – Case Manager

Location: Nyack, NY 10960

Schedule: Monday to Friday | 8-hour shifts

Shift timing: 8:00 AM – 4:30 PM or 8:30 AM – 5:00 PM
Hours/week: 40 hours per week | No weekends
Start Date: 06/01/2026

*Acute care Hospital experience is required.*

Position Overview:

We are seeking an experienced Registered Nurse (RN) Case Manager to join an acute care setting. The ideal candidate will coordinate patient care, ensure appropriate utilization of resources, and support patients through their healthcare journey.

Key Responsibilities:

• Coordinate and manage patient care plans in an acute care environment
• Perform utilization review and discharge planning
• Collaborate with physicians, nurses, and interdisciplinary teams
• Ensure compliance with healthcare regulations and standards
• Monitor patient progress and adjust care plans as needed
• Maintain accurate documentation using EPIC charting system
• Advocate for patients to ensure quality care and appropriate resource use

Qualifications:

• Active RN license (required)
• Minimum 2 years of Case Management experience
• AHA - BLS
• Acute care Hospital experience is required.
• Experience in acute care setting
• Familiarity with EPIC charting system
