Job Title: Licensed Master Social Worker (LMSW) / RN Case Manager

Facility: Tuba City Regional Health Care Corporation
Location: Tuba City
Department: Case Management
Employment Type: Full-Time
Shift: Day / Evening / Night Rotational
Salary Range: $56,935 – $85,253 annually
 

Position Summary

We are seeking a dedicated LMSW / RN Case Manager to join the Case Management team. This role is responsible for coordinating comprehensive, patient-centered care for high-risk populations in an outpatient setting. The candidate will focus on care planning, resource coordination, utilization review, and ensuring continuity of care across multidisciplinary teams.

You will play a key role in improving patient outcomes, reducing care fragmentation, and promoting cost-effective healthcare delivery while empowering patients toward self-sufficiency.
 

Key Responsibilities

  • Conduct patient assessments and develop individualized care plans
  • Coordinate services including referrals, discharge planning, and community resources
  • Monitor patient progress and adjust care plans as needed
  • Perform utilization review and evaluate medical necessity of referrals
  • Collaborate with physicians, nurses, social workers, and external providers
  • Facilitate communication between patients, families, and care teams
  • Manage outpatient referrals, including Purchased Referred Care (PRC)
  • Support transitions of care and ensure continuity post-discharge
  • Maintain accurate documentation in electronic health record systems
  • Promote evidence-based care and patient-centered practices
  • Participate in program development, reporting, and quality improvement initiatives

Required Qualifications

Education

  • For LMSW: Master’s Degree in Social Work (MSW)
  • For RN Case Manager: Associate Degree in Nursing (BSN required within 2 years of hire)

Licensure & Certification

  • Current and unrestricted LMSW/LCSW or RN License (U.S. State or Territory)
  • BLS Certification (American Heart Association)

Experience

  • Minimum 3 years of clinical experience
  • Experience in case management, discharge planning, or utilization review
  • Familiarity with electronic health records (EHR systems)

Preferred Qualifications

  • Bachelor’s Degree in Nursing (BSN)
  • Case Management Certification
  • Experience in hospital or outpatient clinic settings
  • Knowledge of Medicare, Medicaid, or AHCCCS
  • Experience working with diverse or underserved populations
  • Familiarity with Patient-Centered Medical Home (PCMH) model
  • Bilingual skills (Navajo, Hopi, or San Juan Southern Paiute)

Work Environment & Physical Requirements

  • Combination of desk work, patient interaction, and team collaboration
  • Ability to sit, stand, walk, and lift up to 50 lbs
  • Exposure to healthcare environments, including infectious diseases
  • Ability to work flexible schedules and handle high-stress situations
