Job Title: Case Management RN

Location: Santa Rosa, CA 95405

Start Date: 06/01/2026

Shift & Schedule:

Weekends required: 2 weekends every 6 weeks
No on-call required
May be required to float between like units based on facility needs

Requirements

Experience: Minimum 2 years of Case Management experience required
First-time travelers: Accepted with clinical approval (screen required)
License: Active RN license (state of practice)
Certifications:
- BLS (Basic Life Support) – Required
Charting System: Experience with Epic preferred
Local Candidates: Accepted
RTO: Must be disclosed at time of submission
