RN- CASE MANAGEMENT
DESCRIPTION:
Job Title: Case Management RN
Location: Santa Rosa, CA 95405
Start Date: 06/01/2026
Shift & Schedule:
- Weekends required: 2 weekends every 6 weeks
- No on-call required
- May be required to float between like units based on facility needs
Requirements
- Experience: Minimum 2 years of Case Management experience required
- First-time travelers: Accepted with clinical approval (screen required)
- License: Active RN license (state of practice)
- Certifications:
- BLS (Basic Life Support) – Required
- Charting System: Experience with Epic preferred
- Local Candidates: Accepted
- RTO: Must be disclosed at time of submission
