Job Summary:

We are seeking an Occupational Therapist (OT) to provide resident-centered therapy services. The OT will work closely with interdisciplinary teams to improve functional outcomes and support patient independence in a collaborative care environment.

Key Responsibilities:

• Provide and direct occupational therapy services based on clinical standards
• Evaluate residents and develop individualized treatment plans
• Monitor progress and adjust therapy plans as needed
• Coordinate care with therapy staff and interdisciplinary teams
• Participate in scheduled resident screenings
• Provide updates and feedback to supervisors
• Maintain accurate and timely clinical documentation
• Ensure compliance with state, federal, and regulatory requirements
• Maintain billing documentation as required
• Participate in meetings, training, and facility initiatives
• Travel to additional locations as needed

Requirements:

• Master’s degree in occupational therapy
• Active Occupational Therapist (OT) license in the state of practice
• Strong communication and teamwork skills

Preferred Qualifications:

• Experience in geriatric / older adult rehabilitation
• Experience in SNF, LTACH, or long-term care settings
• Knowledge of state, federal, and accreditation guidelines
