Job Description:
Seeking an experienced RN Case Manager Supervisor/Manager to oversee the RN Case Management unit in a hospital setting. Candidates must have strong supervisory or leadership experience along with a seasoned background in RN Case Management and inpatient clinical care. This role requires strong discharge planning, care coordination, and leadership skills while supporting a multidisciplinary team across multiple patient populations.

Requirements:

• Active Washington RN License
• BSN required
• BLS certification required
• Minimum 5 years RN experience
• 3–5 years leadership/supervisory experience required
• Inpatient Case Management experience required
• First-time travelers will not be considered
• EPIC experience preferred

Patient Population:

• NICU
• Pediatrics
• ER
• Med Surg
• Telemetry
• Oncology
• ICU

Case Load:

• Approximately 15–30 patients depending on staffing needs

Key Responsibilities:

• Supervise RN Case Management team
• Coordinate discharge planning and transitions of care
• Manage Medicare/Medicaid patient cases
• Arrange placements to SNF, LTAC, IP Rehab, Assisted Living, Shelter, and other levels of care
• Support discharge planning for homeless and complex patient populations
• Collaborate with physicians, social workers, and interdisciplinary teams
• Perform utilization review and patient assessments

Department Support:

• Case Management Social Workers
• Case Management Assistants
• Daily support from CM Manager
• Partnered with CM Social Worker

Technology:

• EMR: EPIC preferred

Orientation:
