Travel - Occupational Therapist (Hospice/Home Health)

Location: Portland, OR

Specialty: Occupational Therapist

Contract Length: 13 weeks
Schedule: 40hr/Week, Days, M-F

Travel Package: $3,000/Week

Position Description:

The Occupational Therapist is responsible for performing client evaluations, developing and providing occupational therapy services, and documenting services in accordance with the plan of care developed for each patient.

Requirements:

Master's degree in OT, or Doctor of OT: Required only if obtained license on or after January of 2016, or foreign equivalent.
PT license.
2 years OT experience.
CPR/BLS.
Valid Driver's License.

*Local candidates accepted as well. Rate is $62/Hour.
