Description

Job Title: Registered Nurse – Home Health
Location: Charlottesville (VA)

Job Details

Duration: 13 weeks
Shift: Day shift, 5x8 hours (08:00 AM – 05:00 PM)
Schedule: No call or weekends required (weekend availability is a plus)
Start Date: 05/26/2026
Patient Population: Adults & Geriatrics
Visits per Day: Approximately 5

Responsibilities

Perform comprehensive patient assessments and develop individualized care plans
Provide home health and home infusion care
Administer medications (IM, IV, subcutaneous) and manage infusions
Handle central lines, ports, and peripheral IVs
Conduct OASIS documentation and maintain accurate clinical records
Manage start of care, recertification, resumption of care, and discharge planning
Monitor and manage wounds, pressure ulcers, and wound VAC therapy
Provide patient and family education on treatment, medications, and safety
Coordinate care with interdisciplinary teams
Manage chronic conditions including diabetes (glucometer/insulin pump)
Perform phlebotomy and lab-related tasks
Support respiratory care (oxygen therapy, tracheostomy care, nebulizers)
Manage feeding tubes, ostomies, and catheter care

Requirements

Minimum 2 years of RN experience
Home Health and Home Infusion experience required
Travel experience required
Experience with Epic charting system required
BLS certification required
Strong clinical assessment, documentation, and care coordination skills
