Description
Job Title: Home Care RN
Location: Little Rock, AR 72211
Coverage Area: Pulaski, Saline, Jefferson, Faulkner counties
Work Schedule: Monday–Friday, 8:00 AM–5:00 PM; occasional weekends and on-call as needed
Pay: $54.00-$56.00 per hour
Scheduled pay: Weekly
Benefits: Health, Dental, Vision, Life Insurance, and Short term disability after 90 days
Job Type: Full time W2 13 week contract with opportunity of extension
Home Care RN Responsibilities
- Pulaski, Saline, Jefferson, Faulkner counties
- Perform comprehensive assessments of high-risk patients/members
- Collaborate with primary care providers to implement individualized care plans
- Identify, address, and continuously reassess appropriate and cost-effective levels of care
- Coordinate non-medical support (ex: housing, transportation) to improve treatment-plan compliance
- Coordinate care transitions with physicians, social workers, discharge planners, and other partners
- Engage specialty resources as needed to support best outcomes
- Maintain detailed documentation of clinical, functional, and outcome measures
- Promote health education, illness prevention, and patient advocacy while protecting privacy/confidentiality
Home Care RN Required Skills & Qualifications
- Active Arkansas RN license and/or Compact RN required
- Willing to travel within coverage area: Pulaski, Saline, Jefferson, Faulkner counties
- 1+ year Home Health experience
- OASIS, and Homecare Homebase (HCHB) experience required
