Home Health Physical Therapist (PT) – Medford, Oregon
Setting: Home Health
Shift: 5x8 Days
Start: ASAP
Contract Type: 13 Weeks
Looking to break into Home Health or continue building your HH experience in one of the most scenic parts of the Pacific Northwest? This Home Health PT opportunity with Providence in Medford, Oregon is open to travelers with no prior Home Health experience, making it an excellent transition opportunity for therapists coming from outpatient, acute, SNF, or rehab settings.
This role offers strong support, manageable productivity expectations, and a patient-focused 1:1 care model.
Position Overview
The Physical Therapist will provide skilled home health services to adult and geriatric patients throughout the Medford area and surrounding communities. Therapists will evaluate, treat, and educate patients in their homes with the goal of improving independence, mobility, safety, and overall quality of life.
Position Details
- 1:1 patient care model
- Average of up to 6 patients per day
- Productivity expectation: 30 points
- No on-call requirements
- No weekends required
- Travelers without Home Health experience welcome
- EPIC & Oasis documentation
- Mileage reimbursed at IRS standard rate
- Free parking
Requirements
- Active Oregon PT License or pending license accepted
- BCLS certification required
- Oasis experience preferred but not required
- Must have reliable transportation
- COVID vaccine + one booster required

- Facility does a strong job minimizing drive time and grouping patients geographically
- Occasional driving radius may exceed 50 miles
- Any color scrubs allowed
- Local travelers accepted
