RN-CASE MANAGER
DESCRIPTION:
Case Manager – Acute Care Hospital
Position: Case Manager
Start Date: ASAP
Experience Required: Minimum 2 years of Case Management experience
First-Time Travelers: Accepted with Clinical approval
Schedule: Full-time
Weekend Requirement: 2 weekends every 6 weeks
On-Call: Not required
Floating: Yes, traveler may float between like units based on facility needs
Local Candidates: Accepted
Charting System: Epic
Certifications Required: BLS
Required Skills & Experience
- Minimum 2 years of Case Management experience
- Recent acute care hospital experience required
- Strong knowledge of discharge planning and care coordination
- Experience with utilization review and medical necessity criteria
- Ability to work collaboratively in a multidisciplinary environment
- Excellent communication and critical thinking skills
- Current BLS certification required
Preferred Experience
Candidates with experience in the following settings are highly preferred:
- Ambulatory Care
- Behavioral Health
- Home Health
- Hospice
- Insurance Case Management
- LTAC/Rehab/Skilled Nursing
