Position Overview

Seeking an experienced Registered Nurse (RN) for a Home Health contract assignment with Health Quest Homecare. This position involves providing skilled nursing care to patients in their homes across Ulster and Dutchess Counties. The ideal candidate will be comfortable working independently, managing multiple patient visits daily, and delivering compassionate, high-quality care in a home setting.

This is a field-based role requiring travel throughout the assigned territory. Mileage reimbursement is provided.

Shift Details

• Shift: Day Shift
• Hours: 8:30 AM – 5:00 PM
• Shift Length: 8 paid hours
• Meal Break: 30-minute unpaid lunch
• Schedule: Monday–Friday (expected)
• On-Call: None

Responsibilities

• Perform comprehensive patient assessments in home settings
• Administer skilled nursing care according to physician orders and care plans
• Provide wound care and wound vac management
• Perform PICC line care and dressing changes
• Manage PleurX drains and nephrostomy tubes
• Provide ostomy care and education
• Conduct blood draws and venipuncture
• Monitor and educate patients with CHF, diabetes, and chronic illnesses
• Accurately document visits, treatments, and patient progress
• Coordinate care with physicians, case managers, therapists, and caregivers
• Educate patients and families regarding medications, treatment plans, and disease management
• Maintain compliance with agency policies, HIPAA regulations, and clinical standards
• Safely travel between patient homes throughout assigned counties

Territory Coverage

• Ulster County, NY
• Dutchess County, NY

Estimated travel:

• Approximately 50–75 miles daily

Mileage reimbursement provided.

Required Qualifications

• Active NY Registered Nurse License
• Current BLS Certification
• Minimum 1–3 years of Home Health experience
• At least 1 year of recent clinical nursing experience
• Ability to work independently in various home environments
• Reliable transportation and valid driver’s license
• Strong assessment, communication, and documentation skills

Preferred Qualifications

• Experience using Homecare Homebase (HCHB)
• Prior travel nursing experience
• Strong wound care and chronic disease management background
