Registered Nurse (RN) – Nurse Assessor | Hybrid | New York
📍 Location: Multiple Regions Across New York City
🏥 Position Type: Full-Time | Hybrid
🕒 Schedule: 12 Assessments Per Week
🚗 Travel Required: Local travel within assigned region
Job Overview:
We are seeking experienced and compassionate Registered Nurse (RN) Assessors to join a hybrid care assessment team supporting community-based long-term care services throughout New York City. This role combines remote telehealth responsibilities with in-home patient assessments to evaluate healthcare needs, functional status, and eligibility for services.
This is an excellent opportunity for RNs with strong home health and assessment experience who enjoy autonomy, field-based patient interaction, and flexible hybrid work.
Key Responsibilities:
Conduct comprehensive in-home and telehealth assessments for individuals seeking long-term care services and support.
Evaluate health status, functional abilities, care needs, strengths, and patient preferences.
Review medical documentation, referral forms, and clinical records relevant to each case.
Complete and submit assessments accurately using the UAS-NY online system
Document patient interactions, appointment outcomes, and clinical findings
Collaborate with the Clinical Quality Assurance team regarding non-qualified assessments.
Maintain knowledge of community resources and available support services.
Travel throughout assigned regions to conduct in-home consumer visits.
Ensure compliance with all state regulations, assessment guidelines, and nursing standards.
Requirements:
✔ Active RN License in New York State
✔ New York State Driver’s License or State ID required
✔ Minimum of 2 years of recent home healthcare experience
✔ Strong computer skills including Microsoft Outlook, Word, web-based applications, and Zoom
✔ Ability to conduct assessments independently in both home and telehealth settings
✔ Strong clinical judgment, communication, and documentation skills
Preferred Qualifications:
Bachelor’s degree in nursing or health & human services field.
Recent UAS assessment experience (Roles 40 or 45 preferred).
Experience working with geriatric patients and chronic disease populations.
