Job Description
- Assess, plan, implement, and evaluate patient care plans in consultation with healthcare professionals.
- Provide direct and individualized bedside nursing care to pre- and post-surgery patients.
- Prepare, administer, and record prescribed medications.
- Change dressings, insert catheters, and start IVs.
- Adjust specialized medical equipment as needed; interpret and record measurements.
- Monitor patients' vital signs and report adverse medication reactions promptly.
- Collaborate on discharge planning with the healthcare team.
- Educate patients and families on surgical procedures and post-surgery care.
- Maintain and update patient files, medical charts, and health records.
- Adhere to best nursing practices, standards, and protocols.
Work Schedule
- Approximately 7 hours per day, 37.5 hours per week.
- Care provided in patients' homes.
- Manage 4-8 patients per day.
- Work every other weekend is required.
Required Qualifications
Experience
- Minimum 1 year of Home Health experience within the last year.
- Experience in Acute Care, Post Acute, and Home Care settings.
- Med/Surg experience.
- Post-operative care.
Education
- Associate of Science in Nursing or Associate Degree in Nursing from an accredited institution.
Licensure & Certification
- Valid Registered Nurse (RN) license in New York.
- Basic Life Support (BLS) certification issued by the American Heart Association.
