Job Description

  • Assess, plan, implement, and evaluate patient care plans in consultation with healthcare professionals.
  • Provide direct and individualized bedside nursing care to pre- and post-surgery patients.
  • Prepare, administer, and record prescribed medications.
  • Change dressings, insert catheters, and start IVs.
  • Adjust specialized medical equipment as needed; interpret and record measurements.
  • Monitor patients' vital signs and report adverse medication reactions promptly.
  • Collaborate on discharge planning with the healthcare team.
  • Educate patients and families on surgical procedures and post-surgery care.
  • Maintain and update patient files, medical charts, and health records.
  • Adhere to best nursing practices, standards, and protocols.

Work Schedule

  • Approximately 7 hours per day, 37.5 hours per week.
  • Care provided in patients' homes.
  • Manage 4-8 patients per day.
  • Work every other weekend is required.

Required Qualifications

Experience

  • Minimum 1 year of Home Health experience within the last year.
  • Experience in Acute Care, Post Acute, and Home Care settings.
  • Med/Surg experience.
  • Post-operative care.

Education

  • Associate of Science in Nursing or Associate Degree in Nursing from an accredited institution.

Licensure & Certification

  • Valid Registered Nurse (RN) license in New York.
  • Basic Life Support (BLS) certification issued by the American Heart Association.
