Title: Outpatient Pharmacist - Travel Assignment
Hospital: Providence Hood River Memorial Hospital - Hood River, Oregon
Shift: Day Shift | 4x10s | 7:00 AM – 5:00 PM
Contract Duration: 13-Week Travel Contract
Position Overview
Providence Hood River Memorial Hospital is seeking an experienced Outpatient Pharmacist for a 13-week travel assignment at a respected Critical Access and Level III Trauma Center. This role is ideal for pharmacists with strong outpatient infusion experience who thrive in collaborative clinical environments and are committed to delivering exceptional patient care.
The pharmacist will support medication management, clinical dosing, infusion services, and pharmaceutical care within an outpatient and acute care setting.
Key Responsibilities
- Provide pharmaceutical care and medication management for outpatient and acute care patients
- Review and verify medication orders for accuracy and appropriateness
- Perform clinical dosing and pharmacokinetic monitoring
- Prepare and dispense IV medications and compounded sterile products
- Administer and oversee IV piggyback and IV push medication preparation
- Monitor therapeutic drug levels and patient medication responses
- Collaborate with physicians, nurses, and interdisciplinary healthcare teams
- Maintain compliance with hospital policies, regulatory standards, and patient safety protocols
Required Qualifications
- Active Pharmacist license (Oregon license required; pending accepted at submission)
- Minimum 1 year of pharmacist experience
- Outpatient infusion experience required
- BLS certification required
- Strong IV compounding and infusion therapy knowledge
- Epic charting experience required
- Must have reliable transportation for assignment
Preferred Experience
- Acute care or hospital pharmacy experience
- Clinical dosing and therapeutic monitoring experience
- Travel healthcare experience preferred
Schedule & Assignment Details
- Day Shift: 7:00 AM – 5:00 PM
- 4x10-hour schedule
- No call required
- No weekend requirement
- Block scheduling available
- 13-week assignment
- Expected ratio: 1:20
