Description
Job Title: RN, Care Manager (Home Health)
Requirements:
Schedule:
- Day Shift
- Monday through Friday
- 8:00 AM to 5:00 PM
Education and Experience:
- Bachelor’s Degree in Nursing (BSN) preferred
- Acute care facility or home care experience preferred
Licenses and Certifications:
- Active Registered Nurse (RN) license in the state of practice required
- CPR or BLS certification from an approved vendor required
- Valid Driver’s License required
- Must be at least 21 years of age or older
- Current automobile insurance and reliable transportation required
