Description
Travel RN Hospice Liaison opportunity available in Wisconsin for an experienced nurse passionate about end-of-life care, patient advocacy, and hospital-based care coordination. This 13-week assignment is ideal for a clinically strong RN with hospice or palliative experience who excels in patient assessments, provider collaboration, and guiding families through hospice care decisions in acute care settings.

Responsibilities

• Assess hospital and SNF referrals for hospice appropriateness
• Educate patients, families, and providers on hospice philosophy, benefits, and levels of care
• Support hospice admissions from hospital settings when appropriate
• Collaborate with physicians and interdisciplinary teams on end-of-life care planning
• Provide symptom management guidance for pain, dyspnea, and other end-of-life concerns
• Travel between assigned hospital locations as needed
• Maintain accurate documentation in EPIC

Requirements

• Active RN license required
• BLS certification required
• Minimum 2 years of relevant RN experience required
• Hospice or hospice liaison experience strongly preferred
• EPIC experience required
• Strong communication and provider collaboration skills required
• Must have valid driver’s license, auto insurance, and reliable vehicle
• Comfortable traveling between hospital locations

• 4x10 day shifts
• Monday–Friday schedule with one rotating weekday off
• No weekends
