Job Description
Home Health Case Manager RN- Day Shift - M-F 8am-5pm. Rotating weekends required as apart of the 40 hours. EPIC experience required.
Must have minimum 2 years experience having worked in the submitted unit specialty within the last 3 months.
- Registered Nurse (RN) licensure in the state of practice: Required
- BLS: Required
- Valid Driver's License (DL) and must be at least 21 years of age or older: Required
- Current automobile insurance and a reliable automobile: Required
