Description

Job Description:

3 months 8 hour shifts 2 weekend shifts included Case management - UR - Care Coordination and Discharge Planning MUST HAVE ACUTE HOSPITAL EXPERIENCE IN CM.

Regularly reviewing case progress, tracking appointments, and adjusting treatment plans to meet goals.

Supporting patients and families, advocating for needed resources, and intervening in crises.

Connecting clients with services (medical, legal, social) and coordinating care among a multidisciplinary team.

Work Schedule: Days (9AM-5PM)

Required Documents:

AHA BLS Required

AHA ACLS Preferred

Certified Case Manager (CCM) Required

Current California RN License
