Job Title: Licensed Master Social Worker (LMSW) – Acute Care/Hospital Setting

Location: New York, NY
Contract Length: 13 Weeks
Schedule: 9:00 AM – 5:00 PM | 35 Hours/Week
 

Job Description:

We are seeking an experienced Licensed Master Social Worker (LMSW) for an acute care hospital setting in New York, NY. The ideal candidate will have inpatient hospital experience and be skilled in psychosocial assessments, supportive therapy, discharge planning, and referral management.

 

Responsibilities:

  • Conduct psychosocial assessments for patients and families
  • Provide supportive therapy and counseling services
  • Coordinate discharge planning and community referrals
  • Collaborate with interdisciplinary healthcare teams
  • Assist patients with appropriate resource management and care coordination
  • Maintain accurate and timely documentation

Requirements:

  • Master’s Degree in Social Work required
  • Valid LMSW license issued by NYSED required
  • Minimum 1 year of LMSW experience required
  • Prior inpatient hospital, med/surg, rehab, or acute care experience strongly preferred
  • Strong discharge planning and referral management skills
