RN Case Manager

Location: Echo Cliffs Healthcare Center / Tuba City Regional Health Care Corporation
Department: Case Management
Job Type: Full-Time
FLSA Status: Non-Exempt
Reports To: Director of Care Coordination

 

POSITION SUMMARY

The RN Case Manager is responsible for coordinating comprehensive outpatient case management services for high-risk patients at Echo Cliffs Healthcare Center. The RN Case Manager works collaboratively with physicians, nurses, social workers, utilization review teams, Purchased Referred Care (PRC), and community healthcare providers to ensure continuity of care and effective resource utilization.

This position focuses on patient-centered care coordination, discharge planning, utilization review, patient education, and improving healthcare outcomes while promoting cost-effective use of healthcare services. The RN Case Manager supports patients and families throughout the continuum of care to enhance quality of life and reduce fragmentation of services.

 

KEY RESPONSIBILITIES

  • Conduct patient assessments and identify high-risk patients requiring case management services
  • Develop individualized care plans in collaboration with providers and multidisciplinary healthcare teams
  • Coordinate referrals, specialty care services, and community resources
  • Facilitate discharge planning and continuity of care services
  • Monitor patient progress and update care plans as necessary
  • Collaborate with physicians, nurses, PRC staff, utilization review teams, and external healthcare providers
  • Support utilization review and medical necessity determination processes
  • Educate patients and families regarding treatment plans, resources, and self-care management
  • Assist with outpatient referrals and follow-up care coordination
  • Maintain accurate and timely documentation within electronic health record systems
  • Promote evidence-based care practices and patient advocacy
  • Participate in multidisciplinary care coordination meetings
  • Assist with departmental reports, policy updates, and quality improvement initiatives
  • Participate in weekend on-call coverage as assigned
  • Perform additional duties as assigned 

 

REQUIRED QUALIFICATIONS

Education

  • Associate Degree in Nursing (ADN) required
  • Bachelor of Science in Nursing (BSN) must be obtained within two (2) years of hire 

License & Certification

  • Current unrestricted Registered Nurse (RN) license in any U.S. state or territory
  • Current Basic Life Support (BLS) Certification through the American Heart Association 

Experience

  • Minimum three (3) years of clinical nursing experience
  • Experience in case management, discharge planning, utilization review, or care coordination preferred
  • Knowledge of electronic health record (EHR) systems
  • Strong understanding of healthcare coordination and patient care management 

 

PREFERRED QUALIFICATIONS

  • Bachelor of Science in Nursing (BSN)
  • Case Management Certification (CCM or equivalent)
  • Hospital or clinic case management experience
  • Experience with Medicare, Medicaid, AHCCCS, and PRC processes
  • Knowledge of Patient-Centered Medical Home (PCMH) models
  • Experience working with adult and pediatric populations
  • Experience working with culturally diverse or underserved communities
  • Bilingual skills in Navajo, Hopi, or San Juan Southern Paiute preferred

PHYSICAL REQUIREMENTS

  • Ability to sit, stand, walk, bend, and lift up to 50 pounds occasionally
  • Frequent computer and telephone use
  • Ability to work flexible schedules and manage stressful situations 

 

WORK ENVIRONMENT

May involve exposure to infectious diseases, hazardous materials, and variable environmental conditions. Proper use of Personal Protective Equipment (PPE) is required.
