Licensed Practical Nurse (LPN)
π Role Overview
- Location: Lebanon, NH
- Assignment Length: 5 Weeks
- Shift: Day shift (07:00 AM β 07:30 PM)
- Hours Guaranteed: 36 hours per week minimum
- Supervision: Reports directly to Registered Nurse (RN)
- Setting: Rehabilitative / Geriatric Long-Term Care
π Key Responsibilities
1. Clinical Care & Patient Evaluation
- Data Collection: Gather, record, and track subjective/objective patient health data.
- Condition Monitoring: Identify clinical changes and immediately report updates to the RN.
- Care Planning: Help develop, implement, and adjust individualized patient care goals.
- Treatments: Administer prescribed medications and clinical treatments per physician orders.
2. Discharge & Interdisciplinary Coordination
- Care Meetings: Join Utilization Management and Interdisciplinary Care meetings with the RN.
- Discharge Planning: Coordinate discharge recommendations with doctors, patients, and families.
- Patient Education: Prepare families for discharge through targeted medical education.
- Cross-Department Collaboration: Partner across departments to ensure timely, preference-aligned patient care.
3. Shift Management & Supervision
- Staff Oversight: Assign, monitor, and evaluate tasks given to unlicensed assistive personnel (UAP).
- Documentation: Use PointClickCare (PCC) software following standard business protocols.
- Shift Handover: Conduct shift-to-shift reports and mandatory narcotic drug counts.
- Physician Support: Assist attending physicians during patient rounds and clinical examinations.
- Team Culture: Orient new staff, promote safety, and work extended shifts when required.
4. Compliance & Ethics
- Data Privacy: Maintain strict confidentiality of Protected Health Information (PHI) under HIPAA.
- Regulatory Compliance: Adhere to the Federal False Claims Act and facility Codes of Conduct.
- Incident Reporting: Report non-compliance issues immediately via the corporate Integrity Hotline.
- Quality Assurance: Participate actively in facility audits and process improvement workflows.
π Qualifications & Requirements
Educational & Licensing
- Education: Graduate of an accredited School of Practical Nursing (LPN/LVN).
- Licensure: Active, unrestricted New Hampshire (or Compact) LPN Nursing License.
- Certifications: Current Basic Life Support (BLS) / CPR certification.
Experience & Skills
- Experience: Background in geriatric or rehabilitative nursing preferred.
- Software: Familiarity with PointClickCare (PCC) electronic health records.
- Clinical Knowledge: Strong grasp of pharmacology, dosages, and emergency symptom management.
- Leadership: Proven ability to delegate tasks and manage unlicensed clinical staff.
- Communication: Full professional proficiency in reading, writing, and speaking English.
- Health Screening: Verifiable Tuberculin Skin Test (TST) required prior to start.
