Position Overview

The Home Health Registered Nurse provides skilled nursing care to home health patients within the assigned coverage area. This role includes completing OASIS visits, managing patient care, and participating in on-call coverage to ensure continuity of care. The RN works independently in the field while collaborating closely with the interdisciplinary care team.

Schedule & Work Details

• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekly Hour Guarantee: 40 hours
• On-Call/Weekend Requirement: Yes
• Coverage Area: 50 mile radius of the office
• Mileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patient
• Pay Rate: $51–$53 per hour

Key Responsibilities

• Provide skilled nursing care to home health patients in their residences
• Complete OASIS assessments and follow up visits in compliance with regulatory and agency standards
• Perform comprehensive patient assessments and develop individualized plans of care
• Coordinate care with physicians, therapists, and other members of the care team
• Accurately document all visits, assessments, and patient interactions in the electronic medical record
• Participate in on-call coverage, responding to patient needs outside of standard business hours as required
• Educate patients and caregivers on diagnoses, medications, and treatment plans
• Ensure compliance with all clinical policies, procedures, and quality standards

Required Qualifications

• Active Registered Nurse (RN) license
• Home health experience
• Ability to work independently and manage a full patient caseload
• Strong clinical assessment and documentation skills

Preferred Qualifications

• Experience completing OASIS visits
• Strong organizational and time management skills
• Excellent communication and patient education abilities
