Description

Position Overview:
We are seeking an experienced Hospice Registered Nurse Case Manager to provide compassionate, high-quality care to patients in the Marshall and Jackson County, Alabama areas. This role is responsible for managing a caseload of hospice patients in a home setting, ensuring comfort, dignity, and comprehensive care. The ideal candidate is highly independent, clinically strong, and experienced in managing all aspects of home hospice care.

 

Schedule & Work Details:

  • Location: Marshall and Jackson County, Alabama (coverage area within approximately 40 miles of Scottsboro office)
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
    • Daily stand-up call at 8:00 AM and stand-down call at 4:00 PM (via Teams)
  • Weekly Guarantee: 40 hours
  • On-Call/Weekend Requirement:
    • Backup on-call: 1 weekend per month and 1 night per week
    • Supporting 7 on / 7 off primary on-call team
  • Mileage Reimbursement: Yes, between first and last patient visits (not from home to first patient or last patient to home)
  • Pay Rate: $51 to $53 per hour

Key Responsibilities:

  • Manage a caseload of hospice patients in a home setting, ensuring all care needs are met
  • Perform patient admissions, recertifications, and death/discharge visits
  • Conduct comprehensive patient assessments and develop individualized plans of care
  • Provide symptom and pain management to ensure patient comfort
  • See up to 6 patients per day while maintaining high-quality care and documentation
  • Complete documentation and charting using Homecare Homebase PointCare system
  • Participate in IDG (Interdisciplinary Group) meetings and complete all required notes
    • In-person IDG meetings every other Wednesday at the Scottsboro office
  • Coordinate care with interdisciplinary team members including physicians, social workers, and chaplains
  • Order medications, medical supplies, and equipment as needed
  • Educate patients and families on disease progression, medications, and end-of-life care
  • Work independently in diverse home environments and adapt to varying patient needs

Required Qualifications:

  • Active Registered Nurse (RN) license (Associate degree or higher)
  • Hospice RN Case Manager experience required
  • Prior home hospice experience required
  • Proficiency with Homecare Homebase (HCHB) PointCare required
  • Strong clinical assessment and critical thinking skills
  • Ability to work independently with minimal supervision
  • Experience managing hospice patients across multiple settings

Preferred Qualifications:

  • Strong experience with admissions in a hospice setting
  • Ability to work in diverse socio-economic home environments
  • Previous LHC Group travel nurse experience
  • Positive, flexible, and patient-centered approach
  • Excellent communication and organizational skills
