Physical Therapist (PT) – Home Health
📍 Pueblo, CO | 13 Weeks
CenterWell Home Health is seeking an experienced PT for a Home Health assignment in Pueblo, Colorado. The PT will evaluate and treat patients in their homes, develop individualized care plans, and help patients improve mobility, function, safety, and independence.
Position Details
- 13-Week Contract
- Monday-Friday, 8:00 AM – 5:00 PM
- Estimated Gross Pay: ~$2,846/week
- Taxable Rate: $40/hr
- Lodging: $110/day
- M&IE: $68/day
Requirements
- Active CO PT License
- Minimum 1 year of Home Health experience required
- HCHB (Homecare Homebase) experience required
- OASIS experience required
- Reliable transportation
