Description
LPN – LTAC / Hospital Float (Levindale Hebrew Geriatric Center & Hospital)
Location: Baltimore, MD
Facility: LifeBridge Health
Unit: 2000-7032 LPN Float
Shift: Night 3x12 (7:00 PM – 7:30 AM)
Schedule: Every other weekend required
Start Date: 06/08/2026
Float Requirement: Yes – multiple units within facility
Job Summary
Travel LPN needed for a hospital-based float position supporting long-term care, subacute rehab, behavioral health, and respiratory-focused units within Levindale Hebrew Geriatric Center & Hospital.
This role requires flexibility to float across multiple settings including LTC households, rehab halls, pulmonary care, ventilator unit, and subacute medical floors. Strong foundational LPN skills and ability to work in high-acuity long-term care environments are required.
Units Floating To
- LTC Households (dementia/behavioral care environments)
- Subacute Rehab (post-op, stroke, cardiac, wound care)
- Memory Care / Behavioral Health LTC
- Pulmonary Care Unit (including trach patients)
- Ventilator Unit (chronic respiratory/vent-dependent patients)
Key Responsibilities
- Provide direct patient care across multiple LTC and subacute units
- Administer medications (oral, IM, SQ, inhalers, etc.)
- Perform wound care, dressing changes, and monitoring
- Manage tracheostomy and respiratory care (including suctioning and oxygen therapy)
- Monitor vitals, intake/output, and blood glucose
- Assist with ADLs, hygiene, mobility, and safety precautions
- Support behavioral health and dementia patient populations
- Document care in Cerner EMR
- Communicate patient status changes to supervising RN/MD
- Follow infection control and isolation protocols
- Support patient admissions, discharges, and transfers
Required Experience
- Minimum 1 year LPN experience required
- Experience in LTAC, SNF, rehab, or hospital float preferred
- Must be comfortable with high-acuity LTC and behavioral health patients
- Vent/trach experience strongly preferred due to unit population
Required Skills
- Medication administration (oral, IM, SQ)
- Wound care and pressure injury prevention
- Tracheostomy care and suctioning
- Oxygen therapy and respiratory support
- Blood glucose monitoring
- Foley catheter care
- IV therapy support/assistance (as allowed scope)
- Behavioral health/de-escalation techniques
- Basic rehab and mobility assistance
- Cerner EMR documentation
Required Certifications
- Active LPN/LVN license (Maryland or Compact)
- BLS (AHA)
Additional Requirements
- Must float across multiple units within facility
- Must work every other weekend
- Must be flexible with assignments (LTC, rehab, ventilator, pulmonary units)
- Strong attendance required (zero tolerance for tardiness/absence policy)
- Must complete virtual onboarding modules prior to start
- References required
