Job Summary:

Seeking an experienced Home Health Therapist to provide skilled in-home patient care for adult med/surg, orthopedic, neuro, cardiac, respiratory, and post-surgical patients. The clinician will independently manage patient visits, provide therapy interventions, patient education, and complete required documentation.

Key Responsibilities:

• Perform skilled home health visits (6–8 visits per day)
• Conduct patient assessments and develop treatment plans
• Provide mobility training, safety education, ADL support, and Home Exercise Programs (HEP)
• Educate patients and families on medications, safety, and injury prevention
• Complete documentation, billing, and OASIS-related charting accurately

Required Skills:

• Assessment and documentation skills
• Cardiopulmonary/circulatory experience
• Ortho and neuro rehabilitation experience
• Experience with mobility devices and adaptive equipment
• Patient/family education and safety training

Patient Population:

• Med/Surg patients
• Cardiac and respiratory cases
• COPD, CHF, Pneumonia
• CVA, Parkinson’s, weakness, falls
• Trauma, AKA/BKA, neuro patients
• Hip/Knee/Shoulder replacements
• Post-surgical and ADL support patients

Requirements:

• Active State License
• Active BLS Certification
• Minimum 1 year Home Health experience required
• OASIS training preferred
• Valid driver’s license and reliable vehicle required
• COVID vaccination required (or approved exemption)
• Flu shot required or signed declination with mandatory mask use
