Vaccination: Will accept religious/medical form 

On call: Yes / Rotation 

Work the floor - yes if there is a call off - but only for an hour or so 

Pay Virginia building, will be on facility's payroll. 

Must have at least 2 years of experience as a DON in LTC/SNF
