Pride Health is hiring a Home Health Physical Therapist to support our client's home health team in Eureka Springs, AR.

Job Details: 
Schedule:  Monday - Friday, 8 AM -4:30 PM
Location & travel radius: Eureka Springs, AR area (50-mile radius)
Job Type: Local or Travel contractors accepted
Contract Length: 13-week contract
Pay Range:  $55.00 to $60.00 an Hour 

(We are offering local and travel both)
*Pay offered is based on experience, expertise, credentialing, and education.  
 
Duties: 

  • Conduct comprehensive patient evaluations to assess functional status, identify therapy needs, and develop individualized treatment plans in collaboration with physicians and the interdisciplinary care team.
  • Deliver evidence-based physical therapy interventions to improve mobility, reduce pain, restore function, and maximize patient independence while ensuring compliance with state practice standards and agency policies.
  • Monitor patient progress, document evaluations and treatment outcomes accurately, and communicate changes in condition or care needs to physicians, nursing staff, and other healthcare professionals in a timely manner.
  • Educate patients, families, caregivers, and healthcare team members on treatment plans, functional limitations, home exercise programs, safety measures, and progress toward rehabilitation goals.
  • Maintain professional and regulatory compliance by participating in care coordination activities, supervising PTAs/home health aides as applicable, attending staff meetings, completing required documentation, and pursuing ongoing clinical education and licensure requirements.

Requirements: 

  • Arkansas Physical Therapist License.
  • Type of certification required: CPR
  • Minimum Education Requirement: Bachelor's Degree
  • Must have qualifications/experience: Home Health and OASIS experience required.
  • What skills/attributes are nice to have, and will set a candidate apart? HCHB experience.
