Description
Job Title: RN Home Health or Home Hospice
Location: Green Bay, WI 54311-8321
Duration: 13 weeks
Shift: 8a-8p, Fridays, Saturdays and Sundays only (Weekend only)
● About Facility:
o This is a Home Hospice position so will go into patient's homes.
o See below for territory coverage, will depend on requisition as to what territory
● Unit Information:
o Minimum Years of Experience required? 2 years, Home Health or Home Hospice experience
o Will you accept a first-time traveler? Yes, if they have case management experience in the home setting.

● Patient Types/Common Diagnoses:
o Failure to thrive, advanced cancers, end stage COPD, end of life, terminally ill, end stage disease processes
o Attend after hour visits that are called into our triage line (PRN visits - symptom management, fall, etc., death visits). Some issues can be resolved via phone and do not require an in-person visit
● Patient Ratios:
o RN: 0-6, depending on call-ins for the night
● Required License, National Certification, Certs:
o BLS, Active Driver's License, State of WI RN License, own vehicle, auto insurance
o Will be provided badge, laptop, cell phone, and trunk kit once on site
● Skills required:
o Comfortable performing focused assessments and critically thinking, comfortable investigating patient agitation issues (when's the last time they urinated? Is their foley crimped? Etc.), exchanging foley, troubleshooting feeding tubes, wound care, post-fall assessment, comfortable with death visits and conversing with medical examiner or coroner, comfortable administering narcotic pain medications (typically patients will have mediation kit at home), identifying potential drug diversions in the home
● Support within the Department:
o Lead RN 24/7 to call if any issues. Will update Lead RN after visits and give report
o Other Hospice SCT members available via phone
● Technology/Equipment:
o EMR: Epic (Rover), Remote Client
o IV Pump: Sapphire (very rare to see in a home)
● Floating: If yes, where?
o May occasionally float slightly outside of their assigned territory if a call comes in that they are closer to and that territory nurse is in another call. Never over an hour away from home territory.
● Orientation (facility and unit) :
o NEO at territory office
o 3 shifts (1 week) with an SCT nurse
● Scheduling:
o Weekend rotation: Every other
o On call? None
o Holiday Expectations: 1 summer/1 winter
o Approval of RTO: to go through manager
o Shift times: 2000-0800, 3 shifts per week
o Schedule cycle: Usually a set schedule; refer to requisition for what schedule is. Manager can discuss more in 2 nd interview

o Scrub Color: Navy blue
o Parking: Free
o Unit Culture: They are very collaborative and really work together to get patients seen in a timely manner
