Description
Physical Therapist (PT) – Home Health
Join a Home Health team serving the Pueblo community while enjoying everything southern Colorado has to offer. This travel PT opportunity provides the chance to work independently, build strong patient relationships, and help patients safely return to daily life in the comfort of their own homes.
Assignment Details:
- Setting: Home Health
- Schedule: Monday–Friday, daytime hours
- Contract Length: 13 weeks
- Guaranteed Hours
- Mileage reimbursement included

Responsibilities:
- Evaluate and treat patients in their homes
- Develop individualized care plans
- Improve mobility, strength, balance, and functional independence
- Collaborate with nursing and interdisciplinary healthcare teams
- Complete timely documentation and patient education
