Title: Discharge Nurse

Duration: 13+ Weeks + High possibility of extension

Location: Los Angeles, CA 90034

Schedule Notes: Monday–Friday | 11:00 AM – 7:30 PM

Job Summary:

We are seeking an experienced Discharge Nurse to coordinate safe and efficient patient discharges while collaborating closely with physicians, case management, nursing staff, and interdisciplinary teams. The ideal candidate will ensure continuity of care, provide patient education, coordinate post-discharge services, and support smooth patient transitions from hospital to home or other care facilities.

Responsibilities:

• Coordinate and facilitate patient discharge planning and transitions of care
• Collaborate with physicians, case managers, social workers, and bedside staff
• Educate patients and families regarding medications, follow-up care, and discharge instructions
• Ensure all discharge documentation is completed accurately and timely
• Arrange home health services, DME, rehabilitation, or community resources as needed
• Monitor discharge barriers and assist in resolving delays
• Maintain compliance with hospital policies, regulatory standards, and patient safety guidelines
• Provide compassionate, patient-centered care throughout the discharge process

Requirements:

• Active RN License
• BLS Certification
• Minimum 1–2 years of recent acute care nursing experience preferred
• Prior discharge planning, case management, med-surg, or care coordination experience preferred
• Strong communication and organizational skills
• EMR experience
