Description
Pride Health is hiring a Physical Therapist to support our client's patients in Batesville, AR (Coverage Areas: 50-mile radius surrounding Batesville, AR). This is a 13-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Job Name: Home Health Physical Therapist
Contract type: Local Contract
Location: Batesville, AR (Coverage Areas: 50-mile radius surrounding Batesville, AR)
Duration: 13-week Contract (Possibilities of extension)
Pay rate: $50-56/hr. On W2 (Pay Rate is based on experience)
Schedule: Mon – Fri (5 days/wk.) 8 AM to 4:30 PM (32-40 hrs. weekly guaranteed)
Mileage: Reimbursement provided (from first patient to last patient)
On Call/ Weekend: No
Job Responsibilities:
- Perform home health PT evaluations, reassessments, and Start of Care (SOC) visits.
- Develop and implement individualized treatment plans based on patient needs and physician orders.
- Provide skilled physical therapy treatment visits when required.
- Complete accurate and timely clinical documentation in HCHB.
- Educate patients and caregivers on treatment plans, safety, mobility, and home exercise programs.
- Assess patient progress and adjust treatment interventions accordingly.
- Collaborate with interdisciplinary care teams to ensure continuity of care.
- Maintain compliance with agency policies, documentation standards, and home health regulations.
Qualifications:
- Current Physical Therapy license in the state of practice.
- Home Health experience required.
- Experience with Homecare Homebase (HCHB) required.
- Current CPR certification.
- Valid driver’s license, auto insurance, and reliable transportation.
