Community Hospice – RN Case Manager

🕒 Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Key Responsibilities

• Manage and coordinate an assigned hospice patient caseload
• Conduct comprehensive nursing assessments and develop individualized care plans
• Provide symptom management and end-of-life care education to patients and caregivers
• Collaborate with interdisciplinary teams including physicians, social workers, chaplains, and aides
• Deliver compassionate nursing care in homes, assisted living facilities, nursing homes, and other residential settings
• Educate families and caregivers regarding disease progression, medications, and comfort measures
• Maintain accurate and timely clinical documentation
• Participate in occasional weekend backup and on-call rotation as needed

Qualifications

• Active Registered Nurse (RN) license
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Compassionate, patient-centered approach to care
• Ability to work independently and collaboratively within an interdisciplinary team
• Hospice or home health experience preferred, but not required

Schedule & Additional Information

• Full-time, weekday schedule: Monday–Friday, 8:00 AM – 5:00 PM
• Occasional backup coverage for weekends and on-call responsibilities
