Description
Community Hospice – RN Case Manager
📍 Grand Junction 81506
🗓 Start Date: June 15, 2026
🕒 Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Key Responsibilities
- Manage and coordinate an assigned hospice patient caseload
- Conduct comprehensive nursing assessments and develop individualized care plans
- Provide symptom management and end-of-life care education to patients and caregivers
- Collaborate with interdisciplinary teams including physicians, social workers, chaplains, and aides
- Deliver compassionate nursing care in homes, assisted living facilities, nursing homes, and other residential settings
- Educate families and caregivers regarding disease progression, medications, and comfort measures
- Maintain accurate and timely clinical documentation
- Participate in occasional weekend backup and on-call rotation as needed
Qualifications
- Active Registered Nurse (RN) license
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Compassionate, patient-centered approach to care
- Ability to work independently and collaboratively within an interdisciplinary team
- Hospice or home health experience preferred, but not required
Schedule & Additional Information
- Full-time, weekday schedule: Monday–Friday, 8:00 AM – 5:00 PM
- Occasional backup coverage for weekends and on-call responsibilities
