Description

Community Hospice – RN Case Manager

📍 Grand Junction, CO 81506
🗓 Start Date: June 29, 2026
🕒 Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Key Responsibilities

Manage and coordinate an assigned hospice patient caseload
Conduct comprehensive nursing assessments and develop individualized care plans
Provide symptom management and end-of-life care education to patients and caregivers
Collaborate with interdisciplinary teams including physicians, social workers, chaplains, and aides
Deliver compassionate nursing care in homes, assisted living facilities, nursing homes, and other residential settings
Educate families and caregivers regarding disease progression, medications, and comfort measures
Maintain accurate and timely clinical documentation
Participate in occasional weekend backup and on-call rotation as needed

Qualifications

Active Registered Nurse (RN) license
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Compassionate, patient-centered approach to care
Ability to work independently and collaboratively within an interdisciplinary team
Hospice or home health experience preferred, but not required

Schedule & Additional Information

Full-time, weekday schedule: Monday–Friday, 8:00 AM – 5:00 PM
Occasional backup coverage for weekends and on-call responsibilities
