Description
Home Health Physical Therapist – Virginia Beach, VA
Join a Home Health team serving the beautiful coastal Virginia Beach area. This assignment offers the opportunity to work independently while providing one-on-one care to patients in their homes throughout the community. Ideal for an experienced Home Health PT who enjoys autonomy, flexibility, and building strong patient relationships.
This contract is with CenterWell Home Health and requires prior Home Health experience with strong OASIS and documentation skills.
Assignment Details
- Home Health Physical Therapist
- 13-week travel contract
- Full-time day schedule
- Territory-based home visits throughout the Virginia Beach area
- Adult and geriatric patient population
- Competitive weekly pay package
Requirements
- Minimum 1 year of recent Home Health PT experience required
- OASIS experience required
- HomeCare HomeBase (HCHB) experience required
- Current Virginia PT license required
- BLS certification required
- Strong independent clinical and time management skills
Responsibilities
- Evaluate and treat patients in the home setting
- Complete OASIS assessments and point-of-care documentation
- Document daily visits in HomeCare HomeBase
- Develop individualized plans of care focused on safety, mobility, and independence
- Coordinate care with physicians, nursing, caregivers, and interdisciplinary teams
- Educate patients and families on mobility and home safety
