Description
Home Health Physical Therapist – Durham, NC
Join a well-established Home Health team providing individualized care throughout the Durham area. This assignment is ideal for a PT who enjoys autonomy, flexible scheduling, and developing meaningful one-on-one relationships with patients in the home setting.
This opportunity is with Amedisys and requires an experienced Home Health therapist who can confidently manage a caseload, complete OASIS documentation, and work efficiently in HomeCare HomeBase.
Assignment Details
- Home Health Physical Therapist
- 13-week travel contract
- Full-time day schedule
- Adult and geriatric patient population
- Territory-based home visits throughout the Durham area

Requirements
- Previous Home Health PT experience required
- OASIS experience required
- HomeCare HomeBase (HCHB) experience required
- Current North Carolina PT license required
- BLS certification required
- Strong independent clinical and organizational skills
Responsibilities
- Evaluate and treat patients in the home setting
- Complete OASIS assessments and point-of-care documentation
- Document daily visits using HomeCare HomeBase
- Develop individualized treatment plans focused on safety, mobility, and functional independence
- Collaborate with physicians, nursing staff, caregivers, and interdisciplinary teams
- Educate patients and families on mobility, fall prevention, and home safety
